In deze keiharde actiefilm reist sociopaat Dylan 'D' Forrester (Scott Eastwood) op advies van zijn therapeut Dr. Alderwood (Mel Gibson), maar tegen zijn voorwaardelijke vrijlating in, per boot af naar de herdenkingsdienst van zijn jongere broer. FBI-agent Shaunessy (Famke Janssen) verdenkt hem echter van moord en zit hem - ook zonder zijn enkelband- continu op de hielen. Maar als een groep meedogenloze huurlingen zich ook plots op Sentinal Island begeeft, verschuiven de machtsverhoudingen wanneer blijkt dat zijn broer duistere geheimen had. Er ontstaat er een explosief kat-en-muisspel wat zijn weerga niet kent.
